Ozzy Osbourne died of cardiac arrest and coronary artery disease, according to the Black Sabbath frontman’s death certificate.

Ozzy Osbourne’s cause of death has been officially confirmed, weeks after the legendary Black Sabbath frontman passed away at the age of 76.

According to a report from The New York Times, Osbourne died from an “out of hospital” cardiac arrest, with contributing factors listed as acute myocardial infarction, coronary artery disease, and Parkinson’s disease with autonomic dysfunction. The death certificate was filed at a London registry by his daughter, Aimée Osbourne, and lists his profession as “Songwriter, Performer and Rock Legend.”

Ozzy Osbourne passed away on July 22 at the age of 76, surrounded by family at his Buckinghamshire estate, just weeks after he performed one final, emotionally charged show with his Black Sabbath bandmates in Birmingham. His farewell appearance at Villa Park marked the first time in two decades that the band’s classic lineup had played together on stage.

In recent years, Ozzy Osbourne’s health had been a matter of public concern. Since revealing his Parkinson’s diagnosis in 2020, the singer had undergone multiple surgeries and spoken candidly about his struggles with mobility and chronic pain.

Despite this, he was determined to take part in the ‘Back to the Beginning’ concert, performing five solo songs – including “Mama, I’m Coming Home” and “Crazy Train” – before rejoining Black Sabbath for a closing rendition of “Paranoid.”

The show itself drew thousands of fans and a long list of music royalty. Ozzy’s daughter Kelly Osbourne became engaged to her partner Sid Wilson backstage, adding another layer of significance to the night. The concert served not only as a farewell to a singular career but also as a communal celebration of the impact he had on heavy music, pop culture and the people around him.

Ozzy was laid to rest last Thursday in a private service on the grounds of his estate. Around 110 guests attended, including close friends, family, and figures from the music world such as Elton John, Yungblud, Metallica’s James Hetfield, and Marilyn Manson. Black Sabbath members were also in attendance, alongside longtime collaborators like guitarist Zakk Wylde and Slipknot front man Corey Taylor.
